Options: Vision, Mission and/or Purpose of OrganizationLIFE Beaver County is a partnership of Lutheran SeniorLife and Heritage Valley Health System and is administered through the Pennsylvania Department of Human Services and the Centers for Medicare and Medicaid Services (CMS). LIFE means Living Independence for the Elderly. LIFE Beaver County provides comprehensive medical care, adult day services and home care at no cost to qualified older adults 55 years of age and older who wish to remain in their own homes or the home of a caregiver as their need for care increases. Participants have a full range of medical, social and rehabilitation services available to them including comprehensive primary and specialty medical care, adult day services, care at home, prescription coverage, meals, recreational programs and transportation to and from the LIFE Center where medical and social needs can be met. LIFE Beaver County also offers a separate adult day care program that can be paid for through private payment or through some programs through the Beaver County Area Agency on Aging.
History of Organization:LIFE Beaver County enrolled its first participant on March 1, 2004. Lutheran SeniorLife also has LIFE Programs in Armstrong, Butler and Lawrence counties.

Beaver County - LIFE programs through Lutheran SeniorLife are also available in Armstrong, Butler, and Lawrence Counties.
Services ProvidedService DescriptionServices are provided at the Adult Day Health Center and in the client's home. Services may include: physician care, nursing care, rehabilitation therapies, prescription coverage, medical equipment, recreational programs, meals, home care, transportation and more.
An interdisciplinary team of health care professionals assesses the participants' needs, develops individual plans of care and provides coordinated services for total care. Most often, services are provided in the Adult Day Health Center but may also include in-home and referral services. If participant requires hospital or nursing home care, LIFE-Beaver County continues to maintain care through contracted providers and by evaluating and monitoring the participant's ongoing condition and care plan.
Monday - Friday 8:30 a.m. - 5:00 p.m. / 24 hour telephone coverage for medical needs
Fees/Cost for ServicesNo fee for those eligible for Medicaid while living in the community. See additional information.
Eligibility RequirementsParticipants must be 55 years of age or older, reside in Beaver County, be certified as medically eligible by the Beaver County Office on Aging, and be safe to live in the community with the support of the LIFE program.
